Intake Form

Please print and fill-out an intake form on the Intake Form Page, and bring it to your colonic appointment. It will save you time.

Policy

We have a 24-hour cancellation and rescheduling policy: if you contact us or cancel less within 24 hours of your scheduled service, you will be charged 60% of the service cost. This is in respect for the therapist's time. You may send someone else in your place for no extra charge. If you simply do not show-up for your scheduled appointment without any notice, you will be charged 100% of the scheduled service cost. When you schedule, we request an operable credit card number to secure your appointment. This card will only be charged if needed, according to the aforementioned policy. Your personal information is secure and protected. Thanks for respecting both our time and your personal commitment to greater wellbeing and health.
